If you would like to request an appointment please log in to the patient portal.

The portal can also be used to pay your bill and to view your health history & medical record. All new patients please log into the patient portal prior to your first appointment.

• Please fill out the health history questionnaire.
• You will then be directed to the forms page. Please sign all forms prior to your appointment. If you have any questions about the forms, you will be able to discuss them prior to your visit.
